About this tool

This online self-assessment was created for providers of supportive housing (both scattered-site and site-based projects) to evaluate their performance, partnerships and agency practices according to the industry standards of Quality Supportive Housing. 

After completing the self-assessment your agency will receive a summary of your score in each of the Quality Supportive Housing Standards.

This assessment is free of charge. If you would like a more detailed report or speak with a consultant about your results, an additional fee may apply.

BEFORE YOU BEGIN 

  • We encourage you to invite multiple stakeholders to be involved in this process 
  • If you have both scattered and single site projects plan to conduct two separate assessments (assessments slightly differ based on model type) 
  • Complete a draft off-line and review with your stakeholders 

 SUBMITTING YOUR RESPONSES 

  • Submit your responses online by selecting Complete on the last section
  • The assessment does not need to be completed in one go, you may save and return (set a date to complete) 
  • Receive your assessment report after submission.  

 REVIEWING YOUR RESULTS 

  • Plan to review your results with your stakeholders, you will revive a summary of your score for each quality indicator 
  • Use the results and resources to implement quality improvements 
  • Connect with CSH for training and technical assistance support
